To schedule an appointment for surgical or medical oncology, call 919-966-9700 or 1-866-869-1856. We will need basic demographic information as well as information relating to the patient's disease, diagnostic workup and, if applicable, treatment.
The nurse coordinator will advise the referring physician as to the types of medical records that we will need to assemble prior to the patient's initial visit. These records may include operative reports, pathology slides, and the patient's most recent diagnostic tests and scans. It is very important that we have all necessary medical records on hand prior to the visit. This allows us to make a preliminary evaluation of the case in advance, which saves the patient time and can reduce the number of return visits s/he has to make. If patients need additional tests prior to their initial visit, the nurse coordinator will discuss this with them.
Please bring the following to your clinic appointment:
- physician's name, address and phone number
- patient identification card if you have been seen before at UNC
- insurance card and any other related forms required by your insurance company
- list of allergies and medications you are taking with the dosages
